Thank you for considering us to assist in your medical recovery and rehabilitation. I would like to take this opportunity to share with you who we are and what we do. The Center at Lincoln was created to help patients recovering from illness or injury to reach their highest level of independence and wellness.

Since The Center is not a hospital there are a few differences that you may notice after arriving here. We are licensed by the state of Colorado as a skilled nursing facility. That means that we are capable of providing care to patients who have had their acute or surgical illness dealt with at the hospital and now are ready to begin the process of achieving wellness prior to going home.

Medicare covers skilled care in a skilled nursing facility (SNF) under certain conditions for a limited time. Skilled care is health care given when you need skilled nursing or rehabilitation staff to manage, observe and evaluate your care. Examples of skilled care include changing sterile dressings and physical therapy. These services must be provided in a Medicare-certified Skilled Nursing Facility. Care that can be given by non-professional staff is not considered skilled care. Medicare covers certain skilled care services that are needed daily on a short-term basis (up to 100 days).

Your physical therapists will address walking, range of motion, strengthening, balance, and management of stairs and/or inclines. Your occupational therapists will address daily tasks that may have become challenging to you.

During your stay here, you will be provided equipment needed to facilitate your rehabilitation. While these items are for your use while you are our guest, we will be sure to get brand new ones ordered for you upon discharge if you are finding them necessary for use during your daily routine after you leave The Center.

As the medical director I would like to take this opportunity to share with you who we are and what we do.

The Center at Lincoln was created to help patients recovering from illness or injury to reach their highest level of independence and wellness. We accept medically stable patients who have had a recent hospital stay and wish to continue their recovery process in a comfortable and private environment.

We do provide care for medically complex patients. Patients that require oxygen, frequent monitoring, have breathing tubes, feeding tubes, have limited mobility or have large wounds are our specialty. The following is a list of just a few of the conditions that we care for and treat at The Center at Lincoln: congestive heart failure, diabetes mellitus, total knee, hip or shoulder arthroplasty, recovery from stroke, recovery from heart attack, infection requiring intravenous antibiotics, recovery from abdominal surgery, complex wounds, pneumonia, cellulitis, recovery from amputation, paraplegia, multiple sclerosis, Parkinson’s disease, geriatric decline, Alzheimer’s dementia, diverticulitis, hypertension, peptic ulcer disease, peripheral vascular disease, kidney failure, deep venous thrombosis, emphysema, asthma, recovery from hip fracture or fall, pelvic fracture and ongoing cancer care. There are literally hundreds more, but I think you get the picture.

Our combination of resort like accommodations, all private rooms with all private baths, gourmet meals, a state of the art gym, and top notch nurses and therapists make The Center at Lincoln one of the highest ranked centers in Colorado. We are working towards a 5 star rating by Medicare. This is the highest ranking that a facility such as ours can receive. We have put all of this together to serve you…our patient and guest.
